NYU Langone’s Center of Excellence for Alzheimer’s Disease

NYU Langone’s Pearl I. Barlow Center for Memory Evaluation and Treatment is designated as a New York State Center of Excellence for Alzheimer’s Disease. We provide integrative, comprehensive, and coordinated medical services for people with Alzheimer’s disease living in the five boroughs of New York City, as well as their families and caregivers. Services are free and are supported in part with a grant from the New York State Department of Health.

Our team includes neurologists, neuropsychologists, a nurse practitioner, social workers, and a medical assistant, many of whom are bilingual. We focus on promoting early diagnosis and treatment of Alzheimer’s disease; strengthening home and community-based living; delaying the institutionalization of individuals with Alzheimer’s and dementia; and maintaining the best quality of life for patients, caregivers, and their families.

Care Management for Alzheimer’s Disease

We provide effective care management from identification of Alzheimer’s disease and related dementias through needs assessment, care planning, advance care planning, case coordination, counseling and consultation, and referrals. Our licensed clinical social workers develop individualized care plans with you and your family in mind—they can meet with you in person or speak with you over the phone to determine your needs and help you and your family make a long-term plan that works for you. A social worker follows up with you every three months, to make sure that your care plan is still working, and to make any adjustments if needed.

We also offer support services for conditions affecting memory and a cognitive remediation program through the Pearl I. Barlow Center for Memory Evaluation and Treatment.

When necessary, we connect people with Alzheimer’s disease to community partners that can provide additional information, assistance, and support, including CaringKind, the Jewish Community Center of Staten Island, the New York City Chapter of the Alzhemier’s Association, the New York Memory Center, Presbyterian Senior Services, and Sunnyside Community Services. We also work closely with NYU Langone’s Alzheimer’s Disease and Related Dementias Family Support Program.
