The Primary Care Information Project (PCIP)

The Primary Care Information Project (PCIP), a bureau of the New York City Department of Health and Mental Hygiene (DOHMH), was founded in 2005 to improve the health of New Yorkers by supporting the adoption and use of Electronic Health Records (EHRs) among the city’s primary care providers, with a focus on medically underserved populations. PCIP has assisted over 4,100 NYC providers to achieve meaningful use of EHRs. In addition, PCIP field staff utilizes the PCIP Quality Improvement Curriculum, which builds on the Chronic Care Model and the Institute for Healthcare Improvement (IHI) Model for Improvement, to support small practices in workflow redesign and quality improvement.  PCIP will provide practice transformation assistance to participating UPN practices.


The Community Health Care Association of New York State (CHCANYS)

The Community Health Care Association of New York State (CHCANYS) is the oldest statewide primary care association in the US. Founded specifically to support the more than 60 FQHCs in New York State, CHCANYS includes a Quality and Technology Initiatives division that provides technical assistance, training and tools to NYS FQHCs to improve healthcare delivery. CHCANYS will support practice transformation efforts among our FQHC practices.
