HealthyHearts NYC

NYU Langone’s HealthyHearts NYC, part of the Department of Population Health’s Section on Tobacco, Alcohol, and Drug Use, partners with small primary care practices to reduce their patients’ risk of heart disease. We work with approximately 300 practices throughout the five boroughs of New York City—all with fewer than 10 full-time providers—helping them transform the way they deliver care using the latest evidence on improving heart health. We are one of seven cooperatives in the EvidenceNOW: Advancing Heart Health in Primary Care initiative of the Agency for Healthcare Research and Quality (AHRQ). The initiative is helping thousands of primary care practices and millions of patients nationwide.

Our Mission

Heart disease is the leading cause of preventable death in the United States, claiming more lives annually than all cancers combined. Nearly half of American adults are at risk of developing heart disease. We can reduce this number by bringing existing evidence-based treatment and prevention strategies to small primary care practices, which is where most Americans go for care.

Currently, existing guidelines for reducing hypertension, cholesterol levels, and smoking—all of which are proven risk factors for heart disease—are not well integrated into small practices. Additionally, these practices often do not have the resources needed to make quality improvements.

HealthyHearts NYC is working to change this, helping practices build crucial infrastructure, such as optimizing their electronic health records and building quality improvement capacity so they are able to implement evidence-based guidelines. We provide them with the support and resources they need to effectively draw on current data and apply them to the care they deliver. We are evaluating our intervention over three years so that, eventually, other practices can adopt it as well.

We are a team of multidisciplinary health services clinicians, researchers, and practice facilitators in the Department of Population Health, the New York City Department of Health and Mental Hygiene’s Primary Care Information Project, and the Community Health Care Association of New York State.

About the Grant

The EvidenceNOW initiative helps thousands of small- and medium-sized primary care practices across the country use the latest evidence to improve the heart health of millions of Americans. It is aligned with the U.S. Department of Health and Human Services’ Million Hearts® campaign, a national effort to prevent 1 million heart attacks and strokes.
