HealthyHearts NYC Team
HealthyHearts NYC brings together a multidisciplinary group of health services researchers from NYU Langone’s Department of Population Health. We partner with healthcare delivery and public health professionals from the New York City Department of Health and Mental Hygiene’s Primary Care Information Project (PCIP) and the Community Health Care Association of New York State (CHCANYS), the state’s primary care association.

New York City Department of Health and Mental Hygiene’s Primary Care Information Project
The mission of the PCIP, a bureau of the New York City Department of Health and Mental Hygiene, is to advance population health by supporting and promoting high-quality primary care and prevention. PCIP assists New York City–based practices, independently owned community health centers, and hospital ambulatory sites with adopting and implementing health information systems, quality improvement, and practice transformation initiatives. Hang Pham-Singer, PharmD, is PCIP’s principal investigator for HealthyHearts NYC.
Community Health Care Association of New York State
The purpose of the CHCANYS is to ensure that all New Yorkers, including those who are medically underserved, have continuous access to high-quality, community-based healthcare services, including a primary care home. CHCANYS defines new directions in primary care delivery at more than 60 Federally Qualified Health Centers with approximately 600 sites throughout the state. CHCANYS’s goal is to provide community health centers with the resources to provide high-quality, community-based primary care to anyone in need regardless of ability to pay. Diane Ferran, MD, MPH, senior director of clinical quality improvement, is CHCANYS’s principal investigator for HealthyHearts NYC.
A dedicated team of 19 practice facilitators, employed by PCIP and CHCANYS, deliver the HealthyHearts NYC intervention to medical practices across New York City. Facilitators have, on average, five years of prior experience delivering practice facilitation or similar quality improvement assistance to medical practices. They represent a diverse spectrum of age, race, ethnicity, and gender, and more than half are proficient in two or more languages.
