I have had a major interest in identifying a novel biomarker for earlier detection of glucose abnormalities than is possible using current criteria. We have found that the 1-hour plasma glucose during a 75-gram oral glucose tolerance test (OGTT) is a sensitive predictor of high-risk people for progression to type 2 diabetes, cardiovascular complications and mortality. Based on many publications emanating from multinational collaborative research, the International Diabetes Federation has recommended the 1-hour OGTT for diagnosing prediabetes and type 2 diabetes.
A related area of research interest is the use of continuous glucose monitoring (CGM) for detecting changes in glucose homeostasis in people at high-risk without prediabetes. We have found in a published pilot study that CGM could predict prediabetes that was confirmed by an oral glucose tolerance test.
