Predicting poor vaccine responders to Hepatitis B vaccination
Hepatitis B vaccines are indicated broadly for the prevention of hepatitis B infection, yet the vaccine is not uniformly effective in all individuals. We need better understanding of the relevant T and B immunology of the vaccine response to understand why some people respond better than others. Recently, an adjuvanted hepatitis B vaccine was approved that leads to better immune responses with fewer doses. Here, we seek to understand 1) what is the immunological difference between vaccine responders and nonresponders, and 2) what is the difference in the immune responses generated by the adjuvanted vaccine and with the nonadjuvanted vaccine? By focusing on the T and B cell responses in peripheral blood, we will study the generation, maintenance, and longevity of the immune response in those who respond well versus those who do not respond well.
Prediction of Recurrent Anterior Shoulder Instability Using the On/Off Track Method and 3D MRI: A Clinical Outcomes and Cost-Effectiveness Study
To improve the preoperative evaluation of bipolar bone loss, we will develop and optimize a new imaging strategy that consists of a new shoulder imaging technique, 3D MRI, and imaging measurement tool, the On/Off Track method, which can be easily implemented into clinical practice. The On/Off Track method is a technique that uses simple measurements of bipolar bone loss to assess the potential for interaction between these osseous injuries to predict recurrent ASI. Bipolar bone loss will be characterized on MRI with 3D models using the On/Off Track method in ASI patients (n=200). Individual bone injuries will also be quantified using prior proposed measurements (glenoid bone loss %; Hill-Sachs diameter involvement) and thresholds. Patients will be followed for 2 years post-surgery. We will also determine the most cost-effective pre-treatment imaging strategy for patients with a history of ASI. A Markov model using data from our findings and the scientific literature will elucidate the cost-effectiveness tradeoffs between our new MRI-based strategy and current imaging-based algorithms to evaluate bipolar bone loss. We hypothesize that our MRI strategy will be more cost effective than current imaging strategies that rely on individual bone injuries.
