Community Service Plan

The Department of Population Health is committed to developing and implementing effective approaches to disease prevention at the community level. As the architects for the Hospitals Center’s Community Service Plan, we have been able to bring that expertise to bear – from within our own Department and across the Medical Center – developing a group of coordinated, evidence-based programs designed to prevent chronic diseases by reducing risk factors for obesity and reducing tobacco use, and to promote healthy women, infants and children through parenting, early childhood, and teen pregnancy prevention programs.

Our work and our partnerships are centered on the Lower East Side and Chinatown in Manhattan, and Sunset Park in Brooklyn.
