Greater New York City Practice Transformation Network
The Greater New York City Practice Transformation Network (GNYC-PTN), led by a team at NYU Langone Medical Center, provides technical assistance support to help equip approximately 1,800 clinicians in the greater New York City region with tools, information, and network support needed to improve quality of care, increase patients’ access to information, and spend health care dollars more wisely. The $7 million project is funded by the Center for Medicare and Medicaid Services’s Transforming Clinical Practice Initiative.
The NYU Langone team is comprised of faculty and staff from the Department of Population Health, the Center for Healthcare Innovation and Delivery Sciences, DataCore and the Clinically Integrated Network. The GNYC-PTN also includes nine federally qualified health centers, clinicians from the NYU Faculty Group Practice, and partnerships with the Community Health Center Association of New York State and the Primary Care Information Project of the New York City Department of Health and Mental Hygiene.
