Pulmonary & Critical Care Medicine Fellowship Inpatient Rotations

As part of NYU Grossman School of Medicine’s Pulmonary and Critical Care Medicine Fellowship, our many inpatient rotations provide fellows with comprehensive training in pulmonary and critical care medicine.

Bellevue Pulmonary Consult Service

During your first-year rotation on the Bellevue Pulmonary Consult Service, you see consults from a variety of inpatient services at NYC Health + Hospitals/Bellevue. Fellows will care for patients with a wide variety of acute and chronic chest diseases, including opportunistic infections accompanying HIV and tuberculosis. They also gain experience with a wide variety of pulmonary procedures such as thoracentesis and chest tube placement. Fellows also spend two to three days per week mastering bronchoscopy with advanced bronchoscopy attendings, developing experience with procedures such as bronchoalveolar lavage, transbronchial biopsy, and endobronchial ultrasound. First-year fellows on this rotation are also responsible for presenting at a once weekly tuberculosis conference.

Bellevue Chest Service

During your second year, you rotate as “chest chief” for the Bellevue Chest Service, where you, along with advanced practice providers, care for an inpatient service of patients with tuberculosis, exacerbations of asthma or chronic obstructive pulmonary disease, lung cancer, or opportunistic infections. You supervise direct patient care and perform procedures and lead case discussions in a weekly thoracic tumor board, chest radiology conferences, and chest service multidisciplinary rounds. You lead and supervise first-year fellows, rotating residents, and medical students, particularly on procedures such as thoracentesis. You will also spend two to three days per week performing bronchoscopy, with a focus on gaining experience with endobronchial ultrasound.

Bellevue Medical Intensive Care Unit

Both first and second-year fellows rotate on the Bellevue medical intensive care unit (ICU) service. As a first-year fellow, you supervise medical house staff, consisting of four pairs of interns and residents. You oversee morning rounds, where you help formulate patient management plans that you and the residents then implement. Under supervision you perform any necessary procedures, including endotracheal intubation. As a second-year fellow, you will rotate primarily with the advanced practice provider team. Second-year fellows are encouraged to take a leading role on rounds, developing their own management plans, with attending supervision.

The patient population of the Bellevue medical ICU includes patients on veno-venous and veno-arterial extracorporeal membrane oxygenation (ECMO), allowing fellows from both years valuable experience managing these patients. Overnight, the Bellevue medical ICU is managed by an overnight intensivist, allowing fellows evenings at home without call responsibilities.

Tisch Hospital Medical Intensive Care Unit

Both first and second-year fellows rotate through the MICU at Tisch Hospital. First-year fellows supervise the medical interns and residents, while second-year fellows rotate with advanced practice providers. Fellows are responsible for all bedside procedures for ICU patients, including endotracheal intubation under supervision of critical care attendings. Second-year fellows will have the opportunity to care for veno-venous (ECMO) patients on the advanced practice provider service. The medical ICU at Tisch Hospital is managed by a separate overnight intensivist.

Tisch Hospital and Kimmel Pavilion Pulmonary Consult Service

Under the supervision of an attending, a first and second-year fellow on the Tisch Hospital rotation evaluate and provide consultative care to patients with a broad mix of pulmonary diseases. The first-year fellow is in charge of consults from the House Staff Pulmonary Service, while the second-year fellow covers consults at Tisch Hospital and Kimmel Pavilion. Fellows care for patients with advanced bronchiectasis, cystic fibrosis, lung cancer, and nontuberculous mycobacterial infections. Additionally, fellows also help lead discussions in weekly clinical case conferences.

Tisch Hospital Pulmonary Hypertension and Lung Transplant

These rotations focus on solid organ transplant with particular attention to the lung and pulmonary hypertension. As a second-year fellow, you have both inpatient and ambulatory exposure with time spent in outpatient clinics for pulmonary hypertension and transplant, alongside the opportunity to perform right heart catheterization in the Cardiac Catheterization Laboratory.

Tisch Hospital Interventional Bronchoscopy

First-year fellows have a bronchoscopy introductory rotation supervised by interventional pulmonology faculty. The rotation is five days a week and focuses on learning basic bronchoscopic procedures such as bronchoalveolar lavage and transbronchial biopsy. Second and third-year fellows also rotate on this rotation, where they focus on mastering those skills as well as developing further experience with more advanced bronchoscopic procedures such as endobronchial ultrasound.

NYU Langone Hospital—Brooklyn Surgical Intensive Care Unit

At NYU Langone Hospital—Brooklyn, you oversee and coordinate a closed surgical ICU. This unit specializes in preoperative assessment and postoperative care of critically ill surgical patients. The building is a level one trauma center, providing fellows experience caring for high level trauma patients.

NYU Langone Hospital—Brooklyn Medical Intensive Care Unit

First year fellows will help supervise a team of residents and interns in the NYU Langone HospitalBrooklyn medical ICU, which is a combined medical and cardiac ICU. Fellows perform and supervise various ICU procedures, including endotracheal intubation, with supervision by a medical intensivist. As the unit is mixed cardiac, fellows will also have an opportunity to help manage patients with invasive hemodynamic support devices, such as intraaortic balloon pumps. Overnight, the unit is managed by an overnight attending, allowing fellows evenings without home call.

NYU Langone Hospital—Brooklyn Pulmonary Consult Service

At NYU Langone Hospital—Brooklyn, first year fellows will see consults for patients with a variety of pulmonary conditions. Fellows are responsible for performing procedures such as thoracentesis and chest tube placement with attending supervision. Fellows also spend one to two mornings per week performing bronchoscopy, including endobronchial ultrasound.

VA NY Harbor Healthcare System Consult Service

The VA NY Harbor Healthcare System is a general referral center for advanced bronchoscopy and endobronchial ultrasound in the management of intrathoracic pathology. During your second year, you rotate on the consult service and work closely with the pulmonary attending, seeing patients in outpatient pulmonary clinic as well as inpatient consults from a variety of services. Fellows on this rotation will perform a variety of bronchoscopic procedures under attending supervision.

VA NY Harbor Healthcare System Medical Intensive Care Unit

As a first-year fellow, you rotate through the medical ICU, where you supervise house staff. You review each case with a pulmonary and critical care medicine attending, direct the diagnostic workup, perform appropriate procedures, and direct therapy.

VA NY Harbor Healthcare System Surgical Intensive Care Unit

In the surgical ICU, you learn about surgical critical care under the supervision of a pulmonary and critical care medicine attending during your first year. You gain exposure to cardiac, general surgery, thoracic, orthopedic, and neurosurgical cases.

You also spend one to two mornings per week rotating with anesthesia in the operating room, performing endotracheal intubation.
