
Diabetes Screening Volunteer Program
The Diabetes Screening Program at the Ronald O. Perelman Department of Emergency Medicine is seeking dedicated volunteers to join our team. Established in 2019, the program aims to support the total health of emergency department (ED) patients and close care gaps. Over the past five years, as of August 2024, the program has screened 37,481 for prediabetes and diabetes across five EDs and connected these patients to appropriate follow-up care.
We are seeking candidates who are currently enrolled as students in college or university, as this opportunity offers a valuable experience for those interested in medicine, healthcare, public health, or related fields. Ideal candidates will possess excellent communication skills, empathy, and a commitment to making a positive impact on the well-being of ED patients. This volunteer position provides a unique chance to contribute to the community’s health while gaining practical experience in patient engagement and healthcare coordination.
Program Requirements
As a Diabetes Screening Program Volunteer, you will work closely with the project manager and program coordinator to support the program’s objectives.
Key responsibilities include the following:
- conducting telephonic outreach or outreach via MyChart messaging to ED patients who have screened positive for prediabetes or diabetes during their recent visit
- connecting these individuals to outpatient follow-up care promptly, ensuring they receive the necessary support and resources for managing their condition effectively
- reminding patients about the importance of follow-up appointments and assisting them in navigating the healthcare system
- tracking data and outcomes in excel or other software programs with periodic reporting
- collaborating with the program team to maintain and advance program goals
